Dan Ochs of Wrycan Inc. has posted several nice tutorials on customizing the output produced by the DITA Open Toolkit PDF2 Plugin: one covers removing page numbers from cross references, and another explains how to modify PDF2 cover pages to include a company logo. Nice job Dan!
